PREPARING TO RETIRE ( CalSTRS)

District employees considering retirement are urged to undertake, well in advance of their retirement date, certain steps that are necessary to establish future benefits related to retirement. The employee is responsible for completing and submitting all necessary forms within the specified time limits. Forms are available as indicated below.

To print forms from the internet, click the links, below. Completed forms should be mailed to the appropriate address as indicated on the form.

Retirement packets include the following:

1. CalSTRS Service retirement Application (Form SR-0059) is included in the CalSTRS 2012 Your Retirement Guide booklet with other forms and instructions necessary to complete the retirement process. The booklet is 56 pages. The entire booklet or portions of the booklet can be downloaded and printed from the CalSTRS website. (CalSTRS Publications)

2. District Certificated Resignation LAUSD Form (8152)

3. District Application for Continuation of Health Benefits LAUSD Form (HI-22)

Retirement Packets are available:

1) For pick-up at the offices listed below, or
2) For mailing upon request by calling the numbers listed below, or
3) For printing from the on-line internet links provided above.
